In the World of Greyhawk campaign setting the Codex of the Infinite Planes is a powerful artifact.

The Codex of the Infinite Planes is a massive book that even two strong men can barely lift (assuming their Strength to be 16, their combined maximum load is 460 lbs or 209 kg). This tome has the ability to open a portal to any plane in existence.
